MOZILLA USER STUDY RELEASE FORM

Mozilla’s Test Pilot Program is conducting user studies to gather qualitative information about use of the Internet and mobile phones with the aim of improving Firefox and our other products and services. We appreciate your help. 

I freely and voluntarily consent to participate in Mozilla user studies. I understand that my participation in these studies is completely voluntary. I also understand that I may withdraw my consent and discontinue my participation at any time.

I give my permission to Mozilla Corporation and its representatives, parent, subsidiaries, agents, and employees (Mozilla) to interview me and to record my image, voice, and activities during my participation in the user studies through photographs, videos, audio recordings or any other method of recording. I give Mozilla permission to use, reproduce, distribute, modify, display, and perform my feedback provided during the user studies (and derivative works created from that feedback) in any manner and for any purpose, in whole or in part, in perpetuity. I also give Mozilla permission (i) to use the recordings made of me (including derivative works of those recordings), as well as my likeness, name, voice, and biographical information, internally for research and product development purposes and (ii) to post all or portions of such recordings (including derivative works) on public websites, provided that only the information about me checked below may be published in connection with the recordings. I understand that once posted publicly these recordings may be reproduced elsewhere by third parties. I also give Mozilla permission to use all or portions of the recordings (including derivative works) in any media for promotional purposes.

I acknowledge that I will not be compensated for these uses, and that Mozilla exclusively owns all rights to the recordings. I waive the right to inspect or approve uses of the recordings, of any associated copy, or of my feedback. I hereby release Mozilla, its representatives, agents, and employees from any claims that may arise from (i) these uses, including claims of defamation, invasion of privacy, or rights of publicity or copyright and (ii) my participation in the user studies, including injury, property damage, loss of income, or any other claim. This release is binding on me, my heirs, assigns, and estate. Mozilla is not obligated to utilize any of the rights granted under this Release.
